Form 8655 Reporting Agent List

The Reporting Agent List should be submitted together with Form 8655 for each employer for whom the submitter will be a reporting agent. If the list is faxed to the IRS, then the IRS will fill in the Name Control for each employer and fax the list back. Having the correct Name Control for each employer is crucial to the electronic filing of employment tax returns. When submitting employers for which you will be a Reporting Agent, be sure the List Type says "Additions". When removing employers for whom you will no longer be a Reporting Agent, be sure the List Type says "Deletions". The list and forms can be faxed to 855-214-7523 or mailed to:
